The Sexual-Health Peptide Class: A Shelf Named by a Side Effect

Every other category on this site was drawn around something the compounds share. The metabolic shelf is sorted by receptor tier. The growth-hormone shelf is sorted by position on one axis. The immune shelf is sorted by direction of effect.

The sexual-health shelf is different. It is sorted by an outcome — and one that nobody was originally looking for. The founding observation in this entire field was recorded as an unexpected finding in a study about tanning.

That is not a piece of trivia. It is the structural fact that explains why the four compounds filed here (PT-141, Melanotan 2, Melanotan 1, Kisspeptin-10) have less in common with each other than almost any other class on the site.

The founding accident

In 1996, Dorr and colleagues published a pilot Phase 1 study of Melanotan-II — the cyclic lactam-bridged α-MSH analog, Ac-Nle-c[Asp-His-D-Phe-Arg-Trp-Lys]-NH2 (Life Sciences 1996;58:1777-1784). The stated purpose was pigmentation. The primary readouts were quantitative reflectance and visual assessment of tanning, and the study reported that MT-II produced increased pigmentation in the face, upper body and buttock.

Buried in the same abstract is the sentence that created an entire drug class: "A stretching and yawning complex appeared to correlate with the onset of spontaneous, penile erections."

That observation was pursued directly. Wessells et al. ran a double-blind, placebo-controlled crossover study of MT-II in ten men with psychogenic erectile dysfunction using real-time RigiScan monitoring (J Urol 1998;160:389-393). Clinically apparent erections developed in eight of ten; mean duration of tip rigidity greater than 80% was 38.0 minutes with MT-II versus 3.0 with placebo. A follow-up in men with organic risk factors (Urology 2000;56:641-646) reproduced the effect and added the finding that mattered most: self-reported sexual desire was significantly higher after MT-II than after placebo, leading the authors to conclude that centrally acting agents warranted further investigation.

Desire, not vascular tone. That conclusion is the direct ancestor of bremelanotide/PT-141, the MC4R-preferring derivative that eventually reached approval — and of the melanocortin system becoming a sexual-medicine target at all.

Read the lineage plainly: a compound designed to make skin darker generated a research programme about desire, because the receptor family it hit is expressed in more than one place. The class label describes what somebody noticed, not what anybody targeted.

Branch one: the melanocortin arm

Three of the four filed compounds are melanocortin peptides, and they differ by receptor selectivity, which is covered in detail in the Melanotan 1 vs Melanotan 2 comparison and not re-argued here.

The short version: Melanotan 1 is a linear 13-mer weighted toward MC1R and melanogenesis. Melanotan 2 is a cyclic 7-mer that is broadly nonselective across MC1R/MC3R/MC4R/MC5R — which is precisely why a pigmentation-directed compound produced central effects. PT-141 is the MC4R-preferring derivative that separated the central branch from the pigmentation branch.

Structurally, MT-II remains an active scaffold in 2026 medicinal chemistry, not a historical artifact. A Journal of Medicinal Chemistry paper (2026;69:18800-18812) reports 14 macrocyclic analogs of 19-23-membered ring size built on MT-II, SHU-9119 and PG-901 scaffolds, identifying new potent and selective hMC4R antagonists alongside one selective agonist. Note the direction: the same scaffold family yields agonists and antagonists depending on ring size and substitution. Selectivity here is receptor-subtype selectivity in the strict sense described in the receptor pharmacology primer — a genuinely different meaning from the "selectivity" claimed on the growth-hormone shelf.

Branch two: the axis arm

Kisspeptin-10 is filed on the same shelf and shares essentially nothing with the melanocortins. It acts at KISS1R on GnRH neurons — the top of the hypothalamic-pituitary-gonadal axis — and its behaviour is governed by temporal pattern, the variable that defines the hormonal class.

The 2026 literature makes that concrete in an unusual way. A cross-sectional study in Clinical Kidney Journal (2026;19:sfag238) measured serum kisspeptin in 69 maintenance haemodialysis patients versus 20 matched controls and found profound hyperkisspeptinemia — a roughly threefold elevation by median — occurring alongside uncoupling of the HPG axis, which the authors interpret as clinically suggestive of central desensitization. The measurement is observational and the mechanism is inferred, not demonstrated. But the direction is instructive: more of the upstream signal, present continuously, tracked with less downstream axis function. That is the pulsatile-versus-continuous principle appearing as a clinical correlation rather than an infusion protocol.

Worth noting what the active registry actually contains. As of today, the recruiting kisspeptin studies on ClinicalTrials.gov (NCT07224438, NCT07224490, NCT05896293 at Massachusetts General; NCT02081924 at Imperial College London) are reproductive-axis physiology — hypothalamic amenorrhoea, idiopathic hypogonadotropic hypogonadism, quantifying GnRH neuronal function, sustained-administration hormone responses. The brain-processing and arousal work covered in the sexual-health research roundup is real, but it is not where the current interventional programme sits. One shelf, two research communities.

Branch three: pulled in by mechanism

Oxytocin is filed under cognitive-mood, but any honest map of this territory includes it, and it introduces a third receptor architecture entirely — a cyclic nonapeptide at OXTR, embedded in a positive-feedback reflex rather than a negative-feedback axis.

So across four or five compounds the class spans: a family of Gs/cAMP melanocortin GPCRs with five subtypes and endogenous antagonists; a class A GPCR sitting at the apex of a three-tier endocrine cascade; and a neurohypophysial hormone with its own receptor and its own loop topology. There is no shared pharmacology here. There is a shared endpoint that different laboratories arrived at from different directions.

What the human file actually contains

A 2026 PRISMA systematic review in Frontiers in Medicine (2026;13:1789809) searched ClinicalTrials.gov for completed interventional pharmacological trials in women with HSDD and found nine that met criteria — mostly Phase 2 and Phase 3 randomized, double-blind, placebo-controlled studies, predominantly in premenopausal women with acquired generalized HSDD, with flibanserin and bremelanotide the most extensively studied agents.

Two findings from that review deserve emphasis. First, the efficacy endpoints are validated patient-reported measures — Female Sexual Function Index desire domain, Female Sexual Distress Scale — and the review notes that endpoint designation and reporting completeness varied across trials. Second, safety data were inconsistently reported.

Both points matter for reading this shelf. When the outcome of interest is a subjective state, the instrument is part of the result, and cross-trial comparison degrades quickly when instruments and endpoint designations differ. This is the same lesson as the evidence hierarchy piece, sharpened: here the surrogate-versus-outcome question is unusually tangled, because the patient-reported measure is the outcome rather than a proxy for one.

Also note what the review's denominator says. Nine completed trials, across the entire pharmacological landscape for one indication, is a small literature.

The documentation angle

These are structurally awkward molecules on a certificate of analysis, and 2026 supplied a directly relevant paper.

An Analytical Methods study (2026;18:6968-6982, doi 10.1039/d6ay01098a) developed a stability-indicating RP-HPLC method for bremelanotide and characterized its degradants by LC-HRMS/MS under ICH acidic, basic, hydrolytic, oxidative, thermal and photolytic stress — opening with the observation that the compound's intrinsic stability and degradation behaviour have not been extensively investigated. The paper's most useful element for a reader of paperwork is that it pairs mass characterization with computational prediction of epimerization.

Epimerization is the failure mode a routine identity line cannot catch. As covered in the MS/MS and peptide mapping piece, D- and L-residues are isobaric: an epimerized species carries exactly the correct mass. It is invisible to the intact-mass spectrum accompanying a typical COA and is not resolved by a purity percentage unless the chromatographic method happens to separate it.

Three more specifics for this shelf:

  • The cyclic lactam bridge in MT-II and PT-141 is a structural feature the sequence letters do not convey, and its integrity is not reported by a purity number.
  • N-terminal acetylation adds 42 Da, so the theoretical mass on the document must be the acetylated molecule, with a des-acetyl species as a plausible near-mass impurity.
  • Melanotan 2 and PT-141 differ at the C-terminus — amide versus free acid, roughly one dalton, and also one unit of net charge — as detailed in the sequence-reading primer and the charge and solubility primer. Two catalog entries separated by 1 Da is a documentation situation that deserves an explicitly stated theoretical mass.

The label is detaching again

The clearest sign that "sexual health" is a label rather than a mechanism is where the approved melanocortin compound is currently being tested.

NCT06565611 is a Palatin Technologies Phase 2, randomized, double-blind, placebo-controlled study of bremelanotide co-administered with tirzepatide for obesity (n=108 estimated, actual start August 2024, primary completion February 2025, status active-not-recruiting). The primary outcome is percent change in body weight.

That is not a pivot. It is the same receptor. MC4R governs energy homeostasis and is also involved in reproduction — a point made explicitly in a 2026 Molecular and Cellular Endocrinology review of MC4R ligands (2026;621:112876), which frames small-molecule MC4R agonists as potentially relevant to obesity and sexual dysfunction from the same pharmacology.

So the compound that entered the world through a pigmentation study, was redirected by an unexpected observation into sexual medicine, and reached approval there, is now being evaluated on the metabolic branch of the receptor it was always hitting. The biology never changed categories. Only the shelf did.

Three questions

Is this class defined by anything at all? By a research question, not a chemical class or a receptor. That is true of several categories on this site, but it is most extreme here — a melanocortin macrocycle and a KISS1R decapeptide share no target, no tier and no pharmacological vocabulary.

Why are the Melanotans filed here rather than under cosmetic? Because of the 1996-2000 observation chain, not because of a design intent. Melanotan 1 in particular is weighted toward MC1R and melanogenesis; its presence on this shelf is a navigation artifact of the same kind as Dermorphin appearing under healing-recovery.

Does an approved drug in the class validate the rest of it? No. Bremelanotide's file is about a specific indication, a specific population and specific patient-reported instruments. It establishes that the MC4R route is druggable. It says nothing about a nonselective analog, and nothing whatsoever about a KISS1R ligand one axis away.
